I just found the same thing on installing Icad-98 on a new machine, I had Rev-D but forgot to install Rev-E which fixed the problem. Rev-D would not print images (or solids) using the Icad-98 Winline driver. It was OK with the plotter's driver but crashed after sending one plot.
Maybe it's a bug which has made a return in later versions.

It could also be down to the printer. Believe it or not, some plotters cannot handle graphics, strange but true.

Is your printer set up as a HPGL type plotter?

Have you checked to see that it can print raster images?

I have a printer in my office which will not on any accounts print graphics from any package.

Just some food for thought.
