Running IntelliCAD PE 11.1. Using CUI to add command buttons to existing toolbars. I drag the command buttons from the Command pane to the toolbar in the upper left pane under the ICAD toolbar I want the command to reside. The command buttons appear under the ICAD toolbars in the pane but do not appear in the toolbar itself after pushing the "apply" button at the bottom of the CUI page. How can I fix this?

If you are following the same procedure, then all I can suggest is an uninstall, reboot & then reinstall.
Don't forget to release the license first!!!
